Details Department: Case Management Schedule: Monday - Friday.
40 Hours a week.
Hospital: Ascension Saint Thomas Location: Onsite - Nashville, TN.
This position requires some traveling to Ascension Saint Thomas hospitals throughout the TN market.
Regular work location is Saint Thomas Midtown in Nashville, TN.

Responsibilities Lead and direct case management services, workflow and resources.
Formulate and implement operational strategies and initiatives in the case management program.
Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.
Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes and issues.
Successfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.
Oversee compliance activities related to CMS, Joint Commission and state requirements in the hospital setting, including the maintenance of hospital/network standardized policies and procedures.
Develop and maintain standardized initiatives related to hospital case management, including LOS, OBS, multidisciplinary rounding, effective discharge planning and hospital social work activities (general and specialized).

Requirements Licensure / Certification / Registration: One or more of the following required: Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.
Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
One or more of the following preferred: Case Manager credentialed from the American Case Management Association (ACMA).
Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
Education: High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
Master's degree preferred.
Work Experience: 5 years of experience required.
10 years of experience preferred.
2 years of leadership or management experience required.
5 years of leadership or management experience preferred.
