Friday, Saturday and Sunday 3:00 pm to 9:00 pm
Job Responsibilities:
Social: This is a husband and wife case; Their son lives there with them and stays upstairs.
They have no pets.
They mostly enjoy watching TV and enjoy reading the paper.
He is a sports enthusiast and loves specifically tennis and Vanderbilt Baseball.
He has only mild memory loss occasionally.
He does try to help take care of his wife.
Wife is a retired professional seamstres & has always been very creative.
She enjoys decorating and doing crafts, watching the Hallmark channel and talking to friends, but has mid-stage dementia, a short attention span, & repeats the same questions.
They both have hearing loss; neither wear aids.
Bob's Diagnoses: Congestive Heart Failure, Hearing Loss, (no aids), Mobility weakness (uses walker) Wife's Diagnoses: Dementia,(mid-stage), Congestive Heart Failure, Hearing Loss, (no aids), Lower Leg Lymphodema.
ADL's:
Mobility- husband moves slowly and appears somewhat feeble, but uses his walker independently for all transfers and walks.
May need assistance in and out of a vehicle.
Has stair lift chair if needs to go upstairs (rarely).
Wife, takes larger steps and walks leaning forward, but can use her walker independently for all transfers and walks.
May need assistance in and out of a vehicle.

Eating/Nutrition- Both can feed themselves, but need meals and snacks planned, prepare Bathing/Dressing- Husband can get a shower independently and shave himself and brush his teeth on his own.
Wife, gets a shower every day.
She needs a full assist for showering and needs encouragement & sometimes several prompts to get motivated.
She needs help into & out of shower.
Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
