Position Description The Community Health Access and Navigation Team (CHANT) program at the Metro Public Health Department (MPHD) is seeking a motivated, detail-oriented, Public Health Nurse 1 position.
The CHANT program at MPHD works to provide comprehensive, coordinated, family-centered services to eligible families in Davidson County.
The primary focus of this position is to provide the Medical Services Lead and CHANT teams with nursing support to facilitate and enhance care coordination for children and families, particularly those with special health care needs.

Typical Duties Evaluate referrals for medical services for Children and Youth with Special Health Care Needs.
Receives referrals from CHANT Call Center, Central Referral, and community agencies, and ensures completeness of referrals.
Performs monthly chart audits with CSS audit tool to ensure compliance with CHANT, Children’s Special Services (CSS) and HIPAA guidelines.
Participates in the development, implementation and evaluation of CHANT program by reviewing reports and making recommendations for more effective and efficient processes and workflow.
Reviews, approves, denies closed CSS applications.
Reviews and approves Family Service Plans.
Reviews and approves the Transition Plan.
Provides follow-up to Newborn Screenings received from the Tennessee Department of Health to ensure that families are aware of potential medical issues of their newborn child.
This will include, Receiving the report, Contacting the family Maintaining NBS logbook, Providing updates to State Lab and Closure of case and file.
Provides Newborn Screening Referral reports to Central Office.
Receives, logs, and places medical records on charts.
Maintains log for CSS clients with Transition Plans.
Identifies, engages, and maintains relationships with community partners by attending county coalition, health council, and community partnership meetings.
Participates in Team Lead meetings as scheduled.
Maintains required certifications to perform as a registered nurse.
Participates in ongoing learning and development opportunities to maintain and enhance skills and knowledge as a public health professional.
Performs other duties as assigned by the Director of CHANT.
Minimum Qualifications Diploma or Associate’s Degree in nursing.
Candidates with Diploma or Associate’s Degree in nursing must establish a documented ability to competently perform all functions of this classification to be considered.
Bachelor’s degree from a state-approved school of nursing preferred.
Licensed as a Registered Nurse in the State of Tennessee.
Applicants holding a Registered Nurse license from a state that participates in the “Enhanced Nurse Licensure Compact” may be hired, but must obtain their Tennessee license within thirty (30) days of hire.
Cardiopulmonary Resuscitation (CPR) Certification.
All public health nurses are required to complete a department approved Certificate in Public Health as a condition of continued employment.
Possess required competencies upon hiring.
Valid class “D” driver’s license, use of personal vehicle, and maintenance of valid personal vehicle insurance as required by Tennessee Law.
Candidates with accreditations earned in a foreign institute are encouraged to apply, however at the present time MPHD is not sponsoring individuals through any of the US Governments employment-based immigration programs.

S.
Degree in a health or related field.
Two (2) years of progressively responsible full-time paid employment as a registered nurse.
A working understanding of integrated service delivery models and basic knowledge of CSS, HUGS and TennCare Kids outreach.
Proficient in working with REDCap or similar data entry systems.
Familiarity with local and state resources.
Public health experience.
