LPN - 2nd SHIFT

```Duties``` - Provide direct patient care, including administering medications, wound care, and monitoring vital signs - Assist with patient assessments and develop care plans - Perform family planning services, including contraceptive counseling and education - Conduct case management for patients with chronic conditions - Utilize electronic health record systems such as Cerner to document patient information and update medical records - Perform venipuncture and collect laboratory specimens as needed - Assist with sonography procedures as directed by healthcare providers - Provide specialized care for patients with dementia or cognitive impairments - Conduct triage assessments to determine the urgency of patient needs - Administer inhalation therapy as prescribed by healthcare providers ```Experience``` - Must have a valid LPN license in [state] - Minimum of [number] years of experience working as an LPN in a healthcare setting - Experience working with children is preferred - Familiarity with medical records management and documentation practices - Proficient in performing venipuncture and collecting laboratory specimens - Knowledge of family planning services and contraceptive methods - Experience in case management for patients with chronic conditions is a plus - Familiarity with Cerner or other electronic health record systems is preferred Note: This job description is not intended to be all-inclusive.
